Oregon Latino Health Coalition (OLHC) is seeking three Full-Time Community Health Workers for the Oregon Health Plan (OHP) Outreach and Enrollment Program. This position is responsible for providing system navigation support, conducting community-based targeted outreach and education, and application assistance to the Latinx community by performing the following duties.
Founded in 2004, OLHC is the only health policy advocacy organization in Oregon dedicated to eliminating Latina and Latino health disparities. Today, with a growing staff we operate three principal programs: Policy Advocacy, Leadership Development and Outreach and Enrollment.

Qualifications:
- Must be bilingual (Spanish/English) and bi-culturally competent.
- Ability to communicate concisely and effectively, both verbally and in writing in Spanish and English.
- Minimum of three years of experience in human services, healthcare, social services or non-profit sector with demonstrated knowledge of working with the Latino community
- Certification as a Community Health Worker preferred, but not required.
- Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
- Ability to write routine reports and correspondence.
- Successful completion of civil, criminal and/or motor vehicle background checks.
Essential Duties:
- Provides health education to local clients on the Oregon Health Plan (OHP).
- Provides health system navigation support and coordinates care with other health systems to better serve clients, including setting up meetings with medical providers, interpretation, and clinical referral services.
- Develops and launches community charlas “community talks” outreach events and workshops on the OHP plan.
- Participates in community health events and other outreach events such as faith based, Latinx stores and businesses, agricultural farms, and other community public places to disseminate information on OHP and other community resources.
- Maintains strict confidentiality and follows HIPAA policies and procedures when accessing protected health information (PHI).
- Records and updates internal records of case numbers, referrals, grant deliverables and prepares monthly reports.
- Attends required monthly collaborative meetings with Oregon Health Authority (OHA) and other partner meetings as needed to establish partnerships.
- Completes mandatory training to stay current with CHW’s certification and obtains OHP assister certification.
- Conduct home visits to support Latinx families in navigating OHP and other health systems support.
- Conduct outreach and education to the general community on OHP and community health resources using OLHC’s social media platforms.
- Promote and distribute culturally appropriate OHA-approved education and marketing materials in the tri-county area where Latinx members reside.
- Develop step-by-step guides to educate the community on how to access their OHP information and include health literacy and plain language information.
- Coordinates and facilitates other health promotion related workshops when needed.
- Communicates the progress on all tasks and relays any urgent information in a timely manner to the Manager.
- Maintains punctual, regular and predictable attendance.
- Works collaboratively in a team environment with a spirit of cooperation.
- Adheres to the Oregon Latino Health Coalition policies and procedures.
COMPENSATION:
- Annual Salary of $40,000
