Oregon Latino Health Coalition (OLHC) is seeking a Full-Time Community Health Worker. This position is responsible for providing services to community members, their families, other household members, and close contacts, who are COVID positive by performing the following duties.

Founded in 2004, OLHC is the only health policy advocacy organization in Oregon dedicated to eliminating Latina and Latino health disparities. From 2004-2011, we were a volunteer-led organization and not until 2012 did we hire our first Executive Director and begin to formally develop our programming. Today, with a growing staff we operate three principal programs: Policy Advocacy, Leadership Development and OHP Outreach and Enrollment.

Qualifications:

  • Must be bilingual (Spanish/English) and bi-culturally competent.
  • Ability to communicate concisely and effectively, both verbally and in writing in Spanish and English.
  • Minimum of three years of experience in human services, healthcare, social services or non-profit sector with demonstrated knowledge of working with the Latino community
  • Certification as a Community Health Worker preferred, but not required.
  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
  • Ability to write routine reports and correspondence.
  • Successful completion of civil, criminal and/or motor vehicle background checks.

 

Essential Duties:

  • Provides culturally centered education on COVID symptoms monitoring and prevention.
  • Provides informal counseling and emotional support to cope with social isolation.
  • Supports navigation and advocacy for various essential social services and resources (e.g., access to food, housing support).
  • Provides health education for individuals/families impacted by COVID and other chronic conditions.
  • Delivers wrap around support to clients and families, including making appointments, financial assistance, and other essential support services (i.e., food, housing).
  • Provides health navigation support and coordinates care with other health systems to better serve clients, including setting up meetings with medical providers, interpretation, and clinical referral services.
  • Presents culturally centered education on COVID-19 symptoms monitoring and prevention.
  • Provides information on local COVID-19 vaccine clinics.
  • Plans and attends community COVID-19 testing and vaccination sites, including on evenings and weekends.
  • Participates in community health events and other outreach events such as faith based, Latinx stores, agricultural farms, and other community public places to disseminate information on COVID vaccines, Oregon Health plan (OHP) and other community resources.
  • Maintains strict confidentiality and follows HIPAA policies and procedures when accessing protected health information (PHI).
  • Records and updates internal records of case numbers, referrals, grant deliverables and prepares monthly reports.
  • Attends required monthly collaborative meetings with Multnomah County Health Department, Oregon Health Authority (OHA) and other partner meetings as needed to establish partnerships.
  • Completes mandatory training to stay current with CHW’s certification.
  • Coordinates and facilitates other health promotion related workshops when needed.
  • Communicates the progress on all tasks and relays any urgent information in a timely manner to the Manager.
  • Maintains punctual, regular, and predictable attendance.
  • Works collaboratively in a team environment with a spirit of cooperation.
  • Adheres to the Oregon Latino Health Coalition policies and procedures.
  • Other duties as assigned.

 

COMPENSATION:

 Annual Salary of $40,000
